Health Access provides brief in-home health education and assistance to pregnant women and families with children ages birth through five years. The primary focus is on health promotion in the areas of well-child exams, immunizations, breast feeding, nutrition, and home safety. The program also provides classes on these health topics and assists families to apply for health insurance, select a primary doctor of their choice, and link with other needed resources in the community. Services are available to all pregnant women and families with young children. The goal of the program is to improve health outcomes for children.
Community health promoters, called promotoras, provide the in-home education. The promotoras and health care coordinator provide health related classes to the community. A public health nurse is also a member of the team and visits families to provide more in-depth health education.
